That means a higher level of clinical observation, better judgment, and a more qualified person in your home.
Wellness Visit
Proactive Care, Peace of Mind
A Wellness Visit is your regular health check-in: a structured visit by one of our registered nurses designed to monitor your overall health, catch early warning signs, and keep you and your family informed. Think of it as having a trusted nurse on your care team who comes to you.
Wellness Visits are ideal for adults managing chronic conditions, patients recently discharged from a hospital or rehab facility, and anyone whose family wants the reassurance of regular professional oversight. They're also a meaningful option for aging parents whose adult children want objective eyes on their health at home.

- Evaluation of fall risk factors in the home and patient condition; safety recommendations; recommendations for durable medical equipment
- Skin & Wound Observation
- Monitoring for early pressure injuries, redness, skin breakdown, or changes requiring attention

- Structured monitoring and education for heart failure, COPD, diabetes, hypertension, and more

- Check-ins after hospital or rehabilitation discharge to reduce readmission risk

Skilled Care Visit
Advanced Clinical Care at Home
A Skilled Care Visit covers the clinical services that legally and professionally require a licensed registered nurse: procedures that are too complex, too consequential, or too technically demanding to be delegated to non-licensed personnel.
For many patients, Skilled Care Visits eliminate the need for repeated clinic or hospital trips for routine clinical tasks. Your nurse comes to you, equipped, prepared, and experienced, so you can focus on healing in the environment where you're most comfortable.
All Skilled Care Visits are performed by experienced registered nurses with demonstrated competency in the specific skills required. We never send a nurse to perform a procedure they haven't performed with confidence many times before.

- Complex Wound Care
- Assessment, cleaning, and specialized dressing of surgical wounds, diabetic foot wounds, pressure ulcers, infected wounds, and wounds requiring advanced management
- PICC Line & Central Line Dressing Changes
- Sterile dressing changes and comprehensive site assessment for peripherally inserted central catheters and central lines
- IV Medication Administration
- Administration of ordered IV antibiotics, anticoagulants, hydration fluids, and other physician-ordered infusions
- Injections (IM & Subcutaneous)
- Administration of insulin, Lovenox, B12, Humira, and other ordered injectable medications
- Urinary Catheter Care & Replacement
- Sterile insertion, replacement, and ongoing care of Foley and suprapubic indwelling urinary catheters
- Ostomy Care & Patient Education
- Assessment, care, and thorough patient and caregiver education for colostomies, ileostomies, and urostomies
- Enteral Tube Feeding Management
- Insertion verification, administration, and management of G-tube and J-tube enteral feeding
- Tracheostomy Care & Suctioning
- Site care, trach tube maintenance, and nasopharyngeal or tracheostomy suctioning for airway management
- Venipuncture / Lab Specimen Collection
- Blood draws for ordered laboratory tests, collected in the home and transported for processing
- Total Parenteral Nutrition (TPN)
- Initiation and initial monitoring of intravenous nutritional therapy delivered through central venous access
- Medication Teaching & Complex Reconciliation
- Structured education on complex multi-drug regimens; comprehensive reconciliation after hospitalization or care transitions
- Post-Surgical Care & Assessment
- Skilled clinical monitoring and wound management following surgical procedures
- Unstable Condition Assessment
- Skilled evaluation of patients with unpredictable or rapidly changing medical conditions requiring professional clinical judgment
Please note: TPN infusions can run 12+ hours. Our nurse will start the infusion and ensure it is running safely, but does not remain for the full duration.
How Payment Works
NW Private Nurse Partners is a direct-pay service. This means you pay us directly: there are no insurance claims to file, no prior authorizations to wait on, and no coverage denials. You choose the care you need, schedule it on your timeline, and pay for exactly what you receive.
You pay by credit or debit card when you book online — securely processed by Stripe, with Apple Pay and Google Pay supported.

- First-Visit Assessment
- A one-time $50 initial assessment and administrative fee applies to every new patient's first visit — your nurse completes a full health history, medication review, and baseline assessment within your visit hour, and it is never charged again. First visit total: $135 for Wellness, $165 for Skilled Care.
Pricing is built on a simple $85-per-hour nursing rate, includes the nurse's travel anywhere in our service area, and has no membership fees or minimums. You see the exact total before you confirm a booking, and there is no charge for a visit cancelled at least 48 hours ahead.
No referral, no authorization, no waiting. Most patients can be seen within days. Home nursing from a licensed RN is usually HSA and FSA eligible; we provide a detailed receipt for every visit.
