1. Cancellation & Scheduling

• A minimum of 48 hours' notice is required to cancel or reschedule a visit without penalty. Cancellations made with less than 48 hours' notice may be subject to a cancellation fee.
• NW Private Nurse Partners reserves the right to reschedule or cancel a visit due to nurse unavailability, inclement weather, or any other circumstance beyond our reasonable control. We will notify you as promptly as possible in such situations.

2. Home Environment & Safety

• The patient or responsible adult must ensure that the home is safe and accessible for the nurse prior to and during the visit.
• All pets must be secured (in a separate room, crate, or outside) for the entirety of the visit, without exception.
• The home environment must be free from known hazards, including but not limited to smoke, weapons, and hostile or threatening individuals.
• Our nurse reserves the right to decline to begin, or to end, any visit if they reasonably determine that the home environment poses a risk to their personal safety or wellbeing. In such cases, a cancellation fee may apply.

3. Scope of Care

• Services are provided by licensed Registered Nurses (RNs) and are intended to complement, not replace, care provided by your physician or other licensed healthcare providers.
• NW Private Nurse Partners services do not constitute a physician-patient relationship and are not a substitute for emergency medical care.
• In the event of a medical emergency, call 911 immediately. NW Private Nurse Partners is not an emergency service.
• Our nurses may decline to perform any service that falls outside their professional scope of practice or that they deem clinically inappropriate.

4. Patient Responsibilities

• NW Private Nurse Partners nurses do not have access to your medical records, physician files, or prior clinical documentation. It is the patient's (or family's) responsibility to provide the nurse with a full and complete medical history during the initial visit, including all current medications, diagnoses, allergies, and recent procedures. Withholding or omitting relevant medical information may affect the quality and safety of care provided.
• The patient or a responsible adult must be present and available for the full duration of the visit.

5. Payment

• NW Private Nurse Partners is a direct-pay service. Payment is collected at the time of booking. We do not bill insurance companies.
• Visits cancelled at least 48 hours in advance are refunded in full. For cancellations with less notice, refunds are at the discretion of NW Private Nurse Partners.

6. Limitation of Liability

• Northwest Private Nurse Partners LLC and its nurses shall not be liable for any indirect, incidental, or consequential damages arising from the provision of, or failure to provide, services.
• Our liability in connection with any single visit shall not exceed the amount paid for that visit.

7. SMS Messaging Program

• Program name: NW Private Nurse Partners Appointment Reminders
• Description: By opting in during the booking process, you consent to receive SMS appointment reminders and care-related messages from NW Private Nurse Partners, including reminders sent 48 hours and 2 hours before your scheduled visit.
• Message frequency: Up to 2 messages per scheduled appointment.
• Message & data rates may apply depending on your mobile carrier and plan.
• To opt out: Reply STOP to any message at any time. You will receive one confirmation message and no further SMS will be sent.

• We do not share your phone number or personal information with third parties for marketing purposes.

8. Changes to These Terms

NW Private Nurse Partners reserves the right to update these terms at any time. The current version will always be available on this page. Continued use of our services following any update constitutes acceptance of the revised terms.
